1.1 AACE, located in Evansville, Indiana, offers med spa and medical services including primary and urgent care, weight loss programs, hormone replacement therapy, Botox and other neurotoxins, dermal fillers, PDO threads, laser services, microneedling, platelet-rich plasma (PRP), facials, Bela MD, chemical peels, skin tag removal, waxing, lash and brow services, permanent makeup and microblading, teeth whitening, tooth gems, and medical-grade skincare products.
1.2 Specific details of each service, including procedures, risks, benefits and pricing, are provided on our website or during consultation. Services are primarily offered to residents of Evansville, Indiana and the surrounding Tri-State area.
2. Appointments and consultations
2.1 Appointments may be made online, by phone or text at 812-483-7153, or in person.
2.2 You may be asked to provide personal and health information for consultation and treatment purposes. This information is handled in accordance with our Privacy Policy and Notice of Privacy Practices.
2.3 We ask for timely notice of cancellations or rescheduling. Late cancellations or missed appointments may result in a cancellation fee or forfeiture of a deposit or promotional offer.
3. Insurance and payment
3.1 We accept most major medical insurance plans for primary care and urgent care visits, excluding Cigna and MHS. Coverage varies by plan; you are responsible for verifying your benefits and for any copays, deductibles, coinsurance or non-covered services.
3.2 Aesthetic, cosmetic, weight loss, hormone and dental-cosmetic services are self-pay and are not billed to insurance. Payment is due at the time of service unless otherwise agreed in writing.
3.3 Prices and promotions are subject to change without notice. Promotional offers (including new-patient specials) are limited to eligible patients, cannot be combined unless stated, and have no cash value.
4. Medical disclaimer

13. Text messaging (A2P) program
13.1 As part of our service, AACE offers an SMS messaging program that may include appointment reminders, scheduling updates, special offers and other relevant information. You may receive several messages per week depending on your activity and preferences.
13.2 Opt out: You can cancel the SMS service at any time by replying STOP. After you send STOP, we will send one message confirming that you have been unsubscribed. To rejoin, simply opt in again as you did initially.
13.3 Help: For assistance with the messaging program, reply HELP or contact us at 812-483-7153. Carriers are not liable for delayed or undelivered messages.
13.4 Message and data rates may apply to messages sent to and from us. Consent to receive text messages is not a condition of purchase.

13.6 This program is subject to the terms and conditions of your mobile carrier.
14. General
14.1 Severability. If any part of these terms is held invalid or unenforceable, the remaining sections remain in effect.
